Before booking an appointment , take a look at my services and policies to make your experience grand. Waiver and Policy Forms NEW CLIENTS: NEW CLIENTS are required to fill a consultation and policy/ intake form the day of appointment. This form is for you to give me insight on the products you use/take that might interact with retention and that you also understand the policies. Extra Rules: come with NO MAKEUP Guest: additional guest/ kids will not be accepted at your appointment Mask requirements: until further notice mask will be required to begin your appointment Booking: Please schedule fills 2-3 weeks ahead of time. If there are any cancellations it will automatically pop back up in the calendar. Deposits are required to book an appointment you will be charged a non refundable deposit when booking. Fee will depend on service you are booking. Deposits are transferable to another date within the month you booked if you reschedule 3 days before your appointment date. Cancellation/ Reschedule 1 day before or day of will result in loss of deposit and you will need to send another deposit for a new booking date. Guarantee Policy: Heavenly Lush takes great pride in their work! If you have any concerns with the service, let us know within 3 days and we will fix it to your satisfaction at no extra charge. Refunds:I do not offer refunds for any service. If you are not satisfied with your results please contact me kindly and we can figure out how to fix it. (Check Guarantee policy, above) Allergies: If you experience an allergic reaction to any products used during the service, please notify us immediately for assistance assessing the situation. If you have experienced allergic reaction in the past please let us know before your appointment. Unsure of any allergies, a patch test can be administered before the appointment. *Refunds are not given for allergic reactions. Cash, Apple Pay or Card are acceptable as payments Only. Cancellation Policy
If you know you will be late, let me know so we know how much lashing time we will have or to reschedule the appointment.
Cancellation policy and Late Arrivals: If you need to cancel or reschedule, we ask that you please reschedule or cancel at least 1 day before the beginning of your appointment or you may be charged a cancellation fee.
Clients have a 10 minute grace period if you are 10+ minutes late you will have appointments canceled as every minute is important to achieve a full set. Remember every minute lost results in less lashing time. As a result of being late, appointments will be canceled and card on file will be charged a fee that varies on appointment type.
No Call no show will be charged 50% fee and result in being blocked from making future appointments.
